Small Diameter Implants for Upper Premolars

A 74-year-old man is pleased with his first implant placement and wants to have 2 more implants placed in the upper premolar area as soon as possible.  The biggest problem with the first implant is low torque, possibly related to low bone density.  The patient has been on diet to control cardiovascular disease.  He is skinny and relatively small in statue.

Bone density at the sites of #4,5 also looks low preop (Fig.1,2).  To make thing worse, the mesiodistal space is so small that 2 small diameter implants have to be used (Fig.3).  To achieve primary stability, the most effective ways are bone expansion, under prep and long implants.

Prepare #15, micro-osteotomes, and 3 mm 1 piece (angled and straight) and 3.5x17 or 20 mm tissue-level implants.  Initial depth is 14 mm (bone level), PA is to be taken and trajectory is to be changed if necessary.

Stent will be available.
