There is periapical radiolucency (Fig.2 *).   The buccal root tips are coronal to the sinus floor, whereas the palatal one apical to the sinus floor.  It should be safe and advantageous for primary stability to raise the sinus floor to a level around the palatal root tip.

A 5.9x10 mm bone-level implant is placed with minor sinus lift (Fig.3).
