Osteotomy starts with a 2 mm pilot drill (3 and 10 mm deep at the sites of #3 and 4, respectively); with a surgical stent, two parallel pins (PP) are fixed in place for PA (Fig.1).

With this PA, we are able to determine whether the PP at #4 is parallel to the root of the neighboring tooth and where the osteotomy is at #3, relative to the sinus floor.

It is assumed that once the distal end of the osteotomy is cut through with a trephine bur, the sinus floor should be able to be lifted with an osteotome.
