CBCT (axial section) reveals that the septum of #3 (coronal aspect) is large enough to hold 4.5 mm implant (Fig.2; illustration after root removal).  In fact osteotomy is created as planned in the septum using drills and taps (Fig.3 after removing 4.5x20 mm tap).  MB: mesiobuccal; DB: distobuccal; P: palatal.

Although the osteotomy (O) perforates the DB wall of the septum (Fig.3), the primary stability of 4.5x20 mm implant will not be affected, since there is wider septal bone apically.
