When the residual roots are removed, the socket is large, single and 11 mm deep.  Tapered osteotomes (2, 3, 4 mm) are used to gain 3 mm new bone apically and lift the sinus floor.  The osteotomy is further increased by using tapered taps (from 4.5 to 7 mm with increasing stability).  Fig.1 shows a 5 mm tap in place, while Fig.2 the socket and the osteotomy (arrowheads) after osteotome and tap application.  B: buccal; M: mesial; P: palatal.
