When the patient returns for the tooth #15 extraction and implant, the mesiobuccal residual root has been exfoliated, while the distobuccal and palatal roots seem to have extruded (Fig.1).  The bone for primary stability seems to be unlimited.  In fact the bone density is high as well.  The mesial portion of the gingiva (G) is intact and thick, which later prevents bone graft placement in the mesial aspect of the implant to be placed.

Long termed edentulism is associated with the mesial shift of the third molar (arrow).
