Bone Expansion or 1-Piece Implants

A 38-year-old woman has a loose FPD in the lower right quadrant (Fig.1).  The retainer at #31 appears to have open margin (Fig.1 ^) or the abutment has caries (Fig.2 arrowheads).  The FPD will be sectioned between #29 and 30.  The abutment at #31 needs at least a crown, while an implant will be placed at #30.  The bone density at the site is low (Fig.1 pink dashed line).  The latter may be due to narrow bone buccolingually.  If the ridge atrophy is moderate (e.g. 4 mm (implant positioners)), bone expansion will be executed (Bone Expander Kit or IBS Kit) and a 3.8, 4.1 or 4.5 mm implant will be placed.  If the atrophy is severe, the ridge top will be reduced until the ridge is ~ 4 mm, two of 2.5 or 3.0 mm 1-piece implants will be placed (Fig.2).  A splinted provisional will be fabricated at #30 and 31.
