Implant As Large As Possible

A 65-year-old man agrees to have an implant at #18 immediately after #17 extraction because of abutment screw loosening at #30.  But the patient refuses #19 extraction.  Since there is severe vertical bone loss at #18 (Fig.1), the implant should be as large as possible and tissue-level (Fig.2, 5 or 6x14 mm).  The abutment is expected to be 5 mm long.  After implant osteointegration, the abutment should be cemented with Panavia V5.  To control infection at #19 temporarily, scale and place Arestin.
