3rd Placement

Four months post removal of failed implant for 2nd time, osteotomy at #31 starts with Magic Split (flapless), followed by 3 mm Magic Expander (ME).  Since the latter tends to drift distal while being tapped, a 1.5 mm drill is used to move the osteotomy mesial before reuse of the 3 mm ME (Fig.1).  Bone expansion and bending (BEB) continues until 4.3 mm ME; a 4.3 mm Magic Drill (MD) is used for depth (13 mm gingival level).  A 4.5x11 mm dummy implant is placed 2 mm subgingival (Fig.2).  Since the buccal crest (Fig.2 white dashed line) is lower than the lingual one (arrows), a shorter, but wider definitive IBS implant is placed with 50 Ncm (Fig.3: 5x9 mm).  Autogenous bone harvested from MD is placed distal to the implant; a 5.5x5.7(4) mm abutment is placed (Fig.3-5).  The surrounding gingiva adapts well to the abutment (Fig.4,5).  No suture is needed.  The immediate provisional dislodges 4 months postop (Fig.6 (5.5 months postop for #28)).
