A 54-year-old man with bruxism requests treatment at #4 and 12 with buccal fistulae (Fig.1-3).  Due to large defects, a large, but short implants (5x7.3 mm) will be used for primary stability.  PRF membranes (x4) and liquid (x1 for sticky bone) will be prepared for sinus lift and buccal defect repair.  With guide, the trajectory of the implants at #4 and 12 is better than that of #18, 19 and 30 (free hand, Fig.4).
