Fig.4 (preop PA): There is a periapical lesion (*) associated with residual roots of the upper 1st bicuspid. N: nasal cavity; S: maxillary sinus.

Fig.5 is magnification of Fig4.  The width of the root is ~ 5 mm; the length is near 14 mm.  To achieve primary stability, the prospective implant should be engaged into new bone 3-4 mm.  A soft tissue level implant is planned (3 mm associated with the gingiva).  Therefore a 5x20 mm implant is planned.  A tapered, instead of cylindrical, implant seems to be a good fit.
