Peter: In fact, you open my eyes. How did you place these two implants so parallel? Used commercial surgical guide? Bone grafting looks terrific, although it is redundant to me. I used free hand drilling and I am very good about that. You need to training yourself. Basically, I need to watch the drill 3 angles, front, buccal and lingual to make sure they are parallel to #12 all the time.  I don't use commercial guide, it costs $$$.  I used little over 1g of bone. It took me about 1.5 h from sitting patient, to raise the flap, drill, lift, inject the bone, place implants, suture, give the instruction and collect the payment. No leaking, I used hydraulic sinus lift so that patient and I would know if there is any water leaking right away. Patient's son in law is ENT specialist. Dr. Wei, Please feel free to post them on your site.  Hydraulic sinus lift is a technique to lift sinus membrane by slow injecting normal saline water little by little and bone graft after crestal approach osteotomy. It is safer and predictable. Peter Thursday, May 2, 2013 1:19 PM

Dear Dr. Tian: Thank you for generosity sharing the wonderful case. I am posting it. I am still impressed by your implant parallelism. It seems to me that my implants are never placed in a right position, nor in right angulations, although I am watching one dimension while my assistant the other (mesiodistal vs. buccolingual).  Xin

We can talk how to get better angulations.  Only you need to check all angulations.  Not rely on others.  Take study casts.  Play themRemember them of all angulations. You will get sense.  Peter Thursday, May 2, 2013 8:43 PM
