When the flaps are raised, the buccal plate is found to be perforated (Fig.2 *); osteotomy is being established in the narrow palatal wall (<).   A narrow implant is anticipated.  There is a more apical perforation when the osteotomy is angled so that the incisal edge at the site is more or less equivalent to that of the central incisors.   An angled 1-piece implant seems to be required.  Note the curvature of the root of the neighboring central incisor and the thin bone buccal to the root.
