Low Density Post Extraction

A 44-year-old woman will return to clinic for #14 implant placement 2.5 months post extraction (Fig.1).  The tooth used to have persistent fistula.  The bone density appears to be low coronally.  To compensate for the sloped sinus floor (arrowheads), start osteotomy as mesial as possible (Fig.2 red arrow) either with Magic Expander or Drill and place an implant as high as possible (Fig.3).  If there is buccal plate defect, place the implant as palatal and deep as possible.  Use Metronidazole routinely for sinus cases.
