After a 2 mm pilot drill is used for 10 mm, a parallel pin enters for 14 mm, suggesting sinus floor perforation (Fig.3).  Subsequent osteotomy is confined to 8 mm (Fig.4).  Note extensive bony defects (Fig.3,4 *).  Following insertion of collagen plug and allograft, autogenous bone for sinus lift, a 5x10 mm implant is placed with ~35 Ncm (insertion torque); larger size of allograft and Osteogen is placed in the major part of the bony defect (Fig.5).  Further placement of the graft fills up the remaining defect (Fig.6), followed by placement of an abutment (6.5x5(4) mm), graft and collagen plug.  Three months postop, the bone graft remains in place (Fig.7 *) by the abutment and the provisional (P).  Complete Fill of Bony Defect Last Next Xin Wei, DDS, PhD, MS 1st edition 05/20/2016, last revision 12/09/2018
