The anatomic deficiency (distal crest defect, Fig.8 * with one thread exposed locally) is going to be restored with bone graft (Fig.9 *) after a 3x14 mm one-piece (1) implant is placed.  Collagen dressing is applied. After modification of the abutment, an immediate provisional is fabricated.  The gingival cuff of this implant is 4 mm.  It turns out to be a little bit too long.  Modification is required before provisional fabrication.  The implant could be placed deeper.  The large incision allows repair should the labial plate perforates. The patient returns for restoration 4 months postop: mild mesial crestal bone resorption (Fig.12), while some distal crestal bone regeneration.  There is no gingival recession (Fig.15) or bone loss (Fig.16) 4 years 3 months post cementation.
