Small Short Implant: High Torque M

A 4.5x7 mm implant is placed at #31 with >50 Ncm due to the dense bone (Fig.1).  The mesiodistal space is so small that a small abutment has to be used (4.5x4(3) mm).  Partially because of the dense bone and partially nervousness, the patient feels pain with infiltration during osteotomy.  The pain reduces when drill pressure decreases.  Bone shaving has to be removed frequently to relieve pressure.  PA is taken after the abutment is torqued to 30 Ncm after cleaning (2 months postop, Fig.2).  In fact it has to be removed due to subgingival margin and inflamed gingiva.  A 5.5x5(3) mm healing abutment is placed.
